The diagnosis of delirium is clinical, with an emphasis on evaluating level of attention. Attention can be evaluated by serial reversal test (such as asking the patient to spell a word backward). The history should include a review of medications the patient takes and information obtained from friends or family. The neurological examination may not show focal signs or may show myoclonus, dysarthria, tremor, motor abnormalities, or asterixis (loss of tone with passive extension of wrist). Laboratory evaluation should include a comprehensive metabolic panel, glucose, BUN, liver function studies, electrolyte levels, a CBC to evaluate for infection, thyroid function studies to evaluate for endocrinopathy, and ammonia to evaluate for hepatic encephalopathy. Arterial blood gas (ABG) or pulse oximetry should be obtained if the patient has a history of lung disease or smoking. Urine toxicology studies in those individuals with a history of drug abuse or at risk for drug abuse should be requested as well. A CT scan of the head or MRI brain scan needs to be performed with the choice of study depending on ease of obtaining and clinical scenario. Other studies to consider depending on the clinical picture include chest radiograph (evaluates for pneumonia), electrocardiograph (ECG) (exclude myocardial infarction or arrhythmia), electroencephalograph (EEG), and lumbar puncture, if there is concern for central nervous system (CNS) infection.
Patients suspected of cardiogenic syncope or any nonepileptic syncope should undergo an evaluation including an ECG, and echocardiography. Event monitoring, with either a 24-hour Holter or more prolonged monitoring for arrhythmias, is often useful. Patients should have serial orthostatic blood pressure measurements to document a decrease in blood pressure or increase in heart rate with postural changes, which is associated with orthostatic syncope.
This 25-year-old woman developed acute symmetric ascending paralysis with progressive involvement of diaphragmatic muscles. Her most immediate problem is impending respiratory failure. The first priority should be to determine the progression of respiratory insufficiency, usually by serially measuring FVC. The negative inspiratory force should also be followed. Oxygen desaturation occurs much too late to be a safe indicator. An FVC less than 15 mL/kg to 20 mL/kg or maximum inspiratory pressure less than 30 cm H2O usually signals imminent need for intubation and mechanical ventilation. After determining the need for intubation, the next priority is to determine the etiology of the weakness. Guillain-Barré syndrome is the most common cause of acute flaccid paralysis in the United States, occurring in 1 to 3 out of every 100,000 people with a bimodal distribution affecting patients 15 to 35 years of age and 50 to 75 years of age. This patient has the classic history of a bacterial or viral gastrointestinal illness 2 to 4 weeks prior to onset of paresthesia and weakness. She possibly had been exposed to poorly cooked meat, which predisposes her to Campylobacter jejuni. Forty percent of Guillain-Barré patients have positive C jejuni serum antibodies and/or stool cultures. Areflexia is a hallmark examination finding, particularly in conjunction with proximal lower extremity weakness with distal sensory changes and ascending progression. Diaphragmatic and cranial nerve muscles can be affected as well, with up to one-third of patients requiring intubation, as well as autonomic involvement causing bradycardia and hypotension.
At least three options should be considered in managing tumors in the posterior fossa: observation and serial imaging, stereotactic radiosurgery, or conventional surgery. Some of these options might be unavailable or unwise for certain tumor types or tumor size. Clearly, the patient that has a large tumor that is producing brainstem compression or obstructive hydrocephalus should not be observed over time and serially imaged. These findings demand immediate attention.

Walter Sullivan is the main antagonist of Silent Hill 4: The Room. He was an infamous serial killer, killing at least nineteen people (including two children), and a religious fanatic, courtesy of the Order's beliefs being imposed upon him as a child. Attempting to fulfill a macabre ritual called the 21 Sacraments, his life's goal was to cleanse the world of its sins.
Despite him being a serial killer, Walter spoke to Henry Townshend in a surprisingly calm and non-threatening manner. Despite chasing Henry and Eileen, attempting to murder them both, he did not hold any hatred or resentment towards them. Walter viewed Henry and Eileen not as enemies, but simply as two more sacrifices needed to complete the 21 Sacraments ritual, showing an odd empathy-lacking attitude towards killing. He also seemed unable to view his victims as anything beyond their roles as his sacrifices, perhaps showing a minor case of narcissism.
10/21 - Eric Walsh: Tenth of the Ten Hearts - The final victim of Walter's initial rampage. Eric worked as a bartender at Bar Southfield. On his birthday, a middle-aged customer told him that the pet store owner was murdered and the murderer was still at large, causing the bar's owner to close the bar early. Eric then went home, only to find Walter waiting for him. Walter shot him in the face and removed his heart.
Fans were thankful to finally find out the identity of the Ice Truck Killer and his relationship to Dexter, the serial killer with a soul, in the Season 1 finale. Somehow Dexter, a serial killer, murdering a serial killer to save someone he loved, proved he was one of the most complicated heroes on the small screen.
